Incident/accident reporting
If you would like to report an accident or incident that you feel that the Health & Safety Specialist, Welfare Officer or Committee should be aware of please complete the below form.
However should you wish to complete the form offline a copy can be downloaded here.
If completed offline, please email this form to the Welfare Officer (welfare@thamesturbo.co.uk) and Club Secretary (secretary@thamesturbo.co.uk) within 24 hours of incident. Remember to be discrete with information recorded and abide by the Data Protection Act.
